Coffee for strep throat

Have you ever had to look up the modified Centor criteria when someone comes to you with a possible strep throat? You will have to look no more once you know the mnemonic “Old CAFE”. The WM is pretty sure it doesn’t exist elsewhere, so it just might be an exclusive one. :)

Old = Age

C = Cervical lymphadenopathy

A = Absence of cough

F = Fever

E = Exudates in the tonsillar region

Here is how to calculate the modified Centor criteria:

Age < 15 = +1 point

Age > 44 = -1 point

All the rest of the “CAFE” criteria = + 1 point

Here is how to use the modified Centor criteria:

-1 to 1 point = No testing, no antibiotics

2 to 3 points = Perform a culture, give antibiotics if culture positive

4 to 5 = Treat empirically with antibiotics +/- culture

Triads, pentads and classic presentations (part 8)

Just when you think there can not be any more classic presentations, another will present itself to you.

  • Fat embolism may occur following a traumatic event such as multiple long bone fractures. It describes a condition where fat globules pass into the vessels, causing the classic triad of:
    • 1) Respiratory failure
    • 2) Cerebral dysfunction (i.e. confusion)
    • 3) Petechial rash. 
  • The Cushing reflex triad  describes the physiological response to increased intracranial pressure and consists of:
    • 1) Bradycardia
    • 2) Hypertension
    • 3) Irregular breathing

Melanoma

The WM is back from summer break and weekly mnemonics will be coming your way. As summer becomes fall and the hours and intensity of sunshine decrease, here’s a reminder of the ABCDE mnemonic for melanoma.

A = Asymmetry

B = Borders are irregular/uneven

C = Colours are variegated within one lesion

D = Diameter is greater than 0.5 cm

E = Evolving; that is, changing in shape or size, or new symptoms such as bleeding, pain or itching
